4. An email will be sent to you. Make sure your ISP will

5. Complete your document. Add the record number.
6. Make a copy of your complete document and a copy of
our email.
7. Copy on only one (1) side of the page.
8. Do not staple the pages together.
9. Send the completed copy of your document and a copy
of our email to:

MAIL - WE WILL NOT GO TO THE POST OFFICE TO SIGN FOR ANY
PACKAGE.
10. When your document is received we will scan it into
a pdf file and post it to the website.
11. We will not return the document. Therefore, make
sure you do not send us the original, only a copy.
12. If the document is not completed properly, or the
complete document is not sent, or a copy of our email is not included, nothing
will be posted. |
